The Service

We have been asked by NHS England to pilot a new ‘Assertive Community Transitions Service’ for the southern part of the East Midlands region (Northamptonshire and Leicestershire). This will help expedite the safe discharge of forensic mental health hospital patients into the East Midlands, and provide an intensive, individualized support programme for them in order to make this transition smooth and to allow their reintegration into the community to work successfully for all.

To do this, a dedicated multi-disciplinary team is being formed to work with and alongside existing support teams including mental health hospital staff, community forensic services, local mental health teams, substance misuse support services, local housing providers, plus other community agencies and third sector providers. The shared aim will be to reduce lengths of hospital stay for patients, unblock any barriers to smooth and effective community step-down, bring support, confidence and self-help tools to patients that will help them settle in their communities, and intensively work with patients and all other stakeholders to minimize any relapses or need for hospital readmission.

The Role

This role will suit an experienced HCPC registered Psychologist. It will be an essential part of the MDT effort to manage and support forensic patients pre and post discharge, giving them the best chance of a settled, purposeful life in their communities.

The Psychologist will contribute to assessments and development of individualized support plans, deliver therapy and interventions identified for specific individuals, supervise others in delivering their interventions, and provide professional support to the receiving community teams and agencies.

Professional expertise will be required for interventions typically including offence-related work, CAT/DBT/CBT, attachment and relationships, anger management, impulse control, distress tolerance, staying well/mental health awareness, psycho-education and social skills development.

There will also be a need to play a part in a Crisis Response service during the working week and potentially occasionally at weekends/evenings.

About You

You will be professionally qualified and registered, with experience of the needs of recovering forensic mental health hospital patients, as well as knowledge of community step-down and the stakeholders and agencies that can make this happen smoothly and sustainably.

Above all, you will be passionate about giving each patient the psycho-education, the social skills, the self-help tools and any other intensive support necessary to help them re-enter their communities successfully and at the earliest time possible for them.

You will need to have a driving license and access to a car for travel through Leicestershire and/or Northamptonshire, with occasional visits to Nottingham too. Car expenses will be reimbursed.
